About Unique Care LLC
Unique Care LLC is a PASA provider dedicated to supporting individuals with intellectual and developmental disabilities (IDD) and the providers who care for them. We focus on person-centered services that respect each person’s dignity, choices, and goals.
Founded in June 2026 by Donna Smith, Unique Care LLC was created to offer a truly unique PASA experience. We listen, adapt, and stand beside both individuals and providers, working to build safe, respectful, and meaningful supports that help people live fuller, more independent lives.
We believe providers deserve a strong company behind them. That’s why we focus on clear communication, reliable support, and real partnership—so providers never feel alone, and the people they serve receive the highest quality of care.

Donna Smith is the CEO of Unique Care LLC. She holds a Master of Science in Mental Health Counseling and a Bachelor of Science in Human Services. For more than half of her life, Donna has supported individuals with intellectual and developmental disabilities, including people with diverse disabilities and mental health disorders, while building strong leadership skills. Compassionate and dedicated, Donna ensures every person is treated with dignity and respect, empowered to choose their community and living arrangement, and given a meaningful voice. As CEO of Unique Care, she is committed to making sure every individual is truly heard.
Program Manager
Dhamya Clark is the Program Manager at Unique Care LLC. She is pursuing a Bachelor of Science in Mental Health and Social Work and has over five years of experience supporting individuals with intellectual and developmental disabilities and people experiencing homelessness. A dedicated advocate and emerging leader, Dhamya is passionate about making a meaningful difference in people’s lives and helping each person access the support they deserve.
Sephora Toko serves as a Director at Unique Care LLC. Holding a bachelor’s degree in Healthcare Administration and a master’s in Healthcare Administration and Finance, while currently pursuing a PhD in Public Health, she leads our team in delivering expert, person-centered support for individuals with intellectual and developmental disabilities.
